Beth yw Clystyrau?
“Mae clwstwr yn dwyn ynghyd yr holl wasanaethau lleol sy'n ymwneud ag iechyd a gofal ar draws ardal ddaearyddol, gan wasanaethu poblogaeth rhwng 25,000 a 100,000 fel arfer. Mae gweithio fel clwstwr yn sicrhau bod gofal yn cael ei gydlynu'n well i hyrwyddo lles unigolion a chymunedau.”
Ynglyn â Rhwydwaith Clwstwr Castell-nedd
Clwstwr Castell-nedd yw un o'r 11 Clwstwr ar draws ardal Bwrdd Iechyd Prifysgol Bae Abertawe. Roedd yn cynnwys wyth practis cyffredinol yng Nghastell-nedd Port Talbot yn cydweithio â phartneriaid o'r gwasanaethau cymdeithasol, y sector gwirfoddol, a bwrdd iechyd Prifysgol Bae Abertawe. Mae'r Clwstwr yn gwasanaethu tua 56,500 o gleifion.
Ein Gweledigaeth
Datblygu cysylltiadau o fewn ein cymuned a fydd yn galluogi gofal amserol a phriodol i'r rhai sydd angen ein gwasanaethau. Cydweithio i sicrhau bod y gwasanaethau hynny'n gynaliadwy ac o'r ansawdd uchaf posibl, a'u bod yn cael eu darparu yn y gymuned lle bynnag y bo modd.
Cynllun Clwstwr Castell-nedd
The partners, which make up Neath Cluster Network have been working together since 2010 to plan and deliver local services. Dr Burge Jones of Dr Wilkes and Partners leads the cluster.
Prevent ill health enabling people to keep themselves well and independent for as long as possible. Develop the range and quality of services that are provided in the community. Ensure services provided by a wide range of health and social care professionals in the community are better co-ordinated to local needs. Improve communication and information sharing between different health, social care and voluntary sector professionals. Facilitate closer working between community based and hospital services, ensuring that patients receive a smooth and safe transition from hospital services to community based services and vice versa

Neath Cluster Pharmacy Technician
Neath Cluster has employed an experienced pharmacy technician to support the GP practices to review patients’ medication. The technician also supports the care homes and provides training to practice staff.
Neath Cluster Young People’s Wellbeing Project
Neath Cluster has funded Bulldogs to provide the Young People’s Wellbeing Project which provides wellbeing support for young people aged 7-18 years old.
The Project is open to young people with low level mental health issues including:
Improving the uptake of the flu vaccine
Flu can be a very debilitating illness and not every patient who is entitled to have the flu jab gets one. In order to increase the uptake of the vaccine, the cluster has:
Patient Engagement
Neath Cluster has worked with Neath Port Talbot Council for Voluntary Service (CVS) to organise a Have your Say event in November 2017 and a Health and Wellbeing Day in October 2018. The events provided patients with the opportunity to find out about services available locally and provide feedback on some of the services they receive.
Following on from the events, Neath Cluster has continued to work with CVS and the first meeting of Neath Cluster Patient Engagement Group was held in February 2019. The Patient Engagement Group aims to bring together patients from the eight practices to look at how practices and patients can work together to improve health and wellbeing.
At the first meeting, patients discussed the key issues affecting health services, put forward some suggestions for improvements and identified some areas where they can work with practices to improve health and wellbeing. It was agreed that the group would meet on a quarterly basis, so it will meet again in early summer 2019.

Atal diabetes rhag cychwyn
Mae'r clwstwr meddygfeydd yng Nghastell-nedd wedi bod yn gweithio tuag at atal diabetes rhag cychwyn. Maent wedi nodi cleifion sydd mewn perygl o ddatblygu'r cyflwr hwn ac wedi eu gwahodd i gael profion gwaed a derbyn cyngor ar ffordd o fyw.

- Gender identity
- Self-harm
- Bullying
- Depression
- Bereavement
- Family relationships/family breakdown.
Improving the uptake of the flu vaccine
Flu can be a very debilitating illness and not every patient who is entitled to have the flu jab gets one. In order to increase the uptake of the vaccine, the cluster has:
- Set up a network of flu champions
- Worked with the voluntary sector and other services to raise awareness of the dangers of getting the flu
- Trained staff to deliver flu messages
- Gone into the homes of housebound patients to administer the flu vaccine
- Reminded patients at risk of the entitlement to a free flu vaccine
